Unable to upload files

I have created a workpage Models of the Principal Musical Forms (Goetschius, Percy) and have added the file. I must have done something wrong, although I cannot imagine what I did wrong. The file did not show up on the workpage and when I tried adding the file once more I got this message-Fatal Error: Unrecognizable page 'Models of the Principal Musical Forms (Goetschius, Percy)'. Can someone tell me what I did wrong.
