There seems to be something wrong with Special:Unusedimages. For example, Image:Beethoven, L.v. - Piano Sonata 04.pdf is shown as unused (also on the image page), but in fact it appears to be linked from Piano Sonata No.4 (Beethoven, Ludwig van) (first entry).

A fix is planned along with the next upgrade (1.11).

By the way, have you noticed similar issues on Wikipedia proper before?

Nope, as far as I remember, there has been no such issue.
